Reports to : Manager, Case Management for system

Position Description : Coordinates cancer service to selected patient populations. Works with patient populations. Works with physicians and other departments to achieve highest quality clinical outcomes making most effective use of resources

Facility Description : Academic medical center, Trauma I

Reports to : CFO

Position Description : Oversight of all aspects Case Management Program including Case Management, Utilization and Denial Management, Transfer Center, Social Services and Clinical Documentation Improvement. Manages staff of 50 FTEs.

Required Education/Experience : RN or LCSW , Masters preferred.

Additional Skills Desired :Minimum 5+ years case management or social work experience. 3+ years director level experience in large hospital, preferably teaching.

Position Description : Manage case management, Utilization Review and Social Services with 4 - 2 RN Case Managers, 1 RN Utilization Review Coordinator and 1 LCSW

Required Education/Experience : BSN - CA license. CCM strongly preferred. 3 years experience in Case Management. 5 years recent experience at Director level.

Position Description : Oversee all case and utilization functions, Social Workers, Pre- Admissions and Bed Placement for two acute care hospitals, including: Heart Center, Diagnostic, Orthopedic, Emergency and Women's Health Services.

Required Education/Experience : BS, RN, Master's preferred with 5+ years case and utilization management experience.
